The landmark report addresses a critical crossroads for the International Private Medical Insurance (IPMI) and assistance sectors, which face a post-pandemic landscape defined by severe global medical inflation and shifting labour markets. The report highlights how the industry is rapidly transitioning from manual administration to intelligent clinical navigation powered by Artificial Intelligence (AI).
Central to the report’s findings is MDabroad’s proactive approach to technological evolution. Under Rosen’s leadership, MDabroad has executed a landmark, multi-year $1 million infrastructure allocation dedicated entirely to building local AI hardware and advanced engineering resources. This strategic investment enables the "Operational AI" required to repair fragmented financial infrastructure layers, contextualize unstructured cross-border medical billing via "OCR 2.0" (using Large Language Models), and improve provider willingness to treat through error-free, accelerated payments.
“In early 2025, we decided to reallocate $1 million USD to focus on AI, hardware, infrastructure, process alignment, compliance evaluation and augmenting our IT and Software Development teams. That’s a real number and a real commitment. Although I can’t divulge the details, it involves a comprehensive realignment of our proprietary CRM, focused on integrating AI into our processes, wherever human intervention can be enhanced with AI processing. The whole process included the coordination of a special software development team, the active participation of the critical areas: Operations, Claims Handling, Case Management, Compliance, our Medical Department, and our CFO. We had in-house software development, third-party risk assessment, third-party compliance evaluations, it was a company-wide shift towards a new horizon” said Scott J. Rosen, CEO of MDabroad.
Crucially, Rosen introduces a vital strategy to combat the risk of "clinician deskilling" as AI moves into the clinical environment. Dubbed "Deliberate Friction," the policy requires MDabroad’s case managers to document their own professional clinical assessments before reviewing AI-generated outputs.
“My answer is deliberate friction. We require case managers to document their own assessment before reviewing AI output on complex cases. We run regular audits comparing human-only decisions to AI-assisted ones. We actively debate cases where the two diverge — and when the human was right and the AI was wrong, we make sure everyone knows it.”
Rosen continues, “The goal isn’t to keep AI out. It’s to keep human expertise sharp enough to know when AI is wrong. Because it will be.”
The report also touches upon the legal and ethical framework of the "Algorithmic Gatekeeper," clarifying that insurers must carry primary moral and legal liability for coverage decisions, while emphasizing the existential need for "Explainability"—articulating claims decisions in plain, human-readable language tied to policy terms rather than opaque code logic.
